Organ quality, not donor supply, limits transplantation.
After circulation stops, warm ischemia can initiate cellular stress and injury that may continue during organ recovery, transport, and reperfusion.
Donation after Cardiocirculatory Death (DCD) organs are often underutilized because of warm ischemia, time-to-recovery constraints, and quality concerns.
Current preservation and perfusion systems help assess organ function, but additional tools may be needed to better support the use of DCD and other high-risk donor organs.
